Fall Prevention Program

 

Falls are a threat to the health of older adults and can reduce their ability to remain independent, but they are preventable.

The Fall Prevention Program provides free screenings, clinical assessments, and home modifications to those with a great risk of falling. Recipients will be assessed by the organization’s Fall Prevention coordinator and physical or occupational therapist to determine fall risk. Once approved, free modifications such as grab bars, improved lighting, and other fall prevention equipment will be provided, though no structural modifications will be made.

PROGRAM REQUIREMENTS

  • Seniors (60+) or Permanently Disabled Adults

  • Resident of Solano County

  • Fear of falling, history of falls, desire to prevent falls

PROGRAM BENEFITS

  • Physical or Occupational Therapy assessment

  • Fall Prevention medical equipment and installation
